If you're reading this webpage, you probably
know that in advance of my upcoming surgeries, in an abundance of
caution, I put together my "Healthcare Advanced Directives" a.k.a. my
Living Will.
I think every adult should consider filling out a Living Will which
makes your wishes known should you become incapacitated and incapable of
making your own decisions.
Living Wills are (apparently) specific for each state, and you can
download your state's free form here:

For Florida residents, and especially my quad friends, I made the
Florida form a fillable PDF file, so you can fill it out via computer
without needing a caregiver to write for you. You can download that free

NOTE #1: If you want to use the above fillable form, after clicking the link and
waiting for it to download, in the upper left corner, click the "save"
button and save it to a folder on your computer. Then from the
folder on your computer, you can open, fill out, and
save the file before printing it. (You can also save it for later
updating should you ever wish or need to change anything.)
NOTE #2: if you'd prefer to handwrite your Florida Living Will, then I
would download the original form

print my "blank" fillable version, however there are a couple spots
where I have info filled in that I intend for you to overwrite or delete
when filling it out via computer.)
